Registration rates are as follows:

ASQDE or SWAFDE Members:  $400 (by July 8), $475 (July 9-29) , $550 (July 30-Aug. 8)
Invited Guests: $500 (by July 8), $600 (July 9-29), $700 (July 30-Aug. 8)
Trainees: $200 (by July 8); $300 (July 9-29), $400  (July 30-Aug. 8)

ASQDE/SWAFDE CONFERENCE REFUND POLICY

All requests for cancellation/refund must be made in writing via email only to asqjournal@gmail.com.

• 100% REFUND (less a $25 administrative fee) for requests received through July 9.
• 75% REFUND for requests received between July 10 and July 15.
• 50% REFUND for requests received between July 16 and July 29.
• NO REFUNDS given as of July 30.

COVID exception: If you test positive for COVID within 5 days of your travel date to the conference, you may request a full refund (less a $25 administrative fee). Please email your request to asqjournal@gmail.com and attach (1) an electronic copy of your test result displaying your full name and date of the test, and (2) a copy of your airline reservation receipt showing the dates of your planned travel to the conference.

NOTE: If you've been exposed to someone with COVID within five days of traveling to the conference, but have tested negative prior to your departure with no symptoms, it is recommended that you  continue to wear a well-fitting mask until the 10th day after exposure.

NON-MEMBERS:
How to request a guest invitation:

• Contact an ASQDE Member or SWAFDE Member who is familiar with your work and ask them to request an invitation on your behalf. 
• If you have attended three prior ASQDE meetings without participating, you will receive a conditional invitation that requires you to submit an abstract and your final presentation in order to attend the meeting.
• If you do not know an ASQDE or SWAFDE Member, you can complete this Non-Member Request for Guest Invitation form, which will require you to upload a PDF of your current Curriculum Vitae. Your request will be reviewed by the ASQDE Executive Committee.

ASQDE/SWAFDE Members ONLY:

• You can use this form to request a guest invitation for a non-member you know.

Approved guests will receive the invitation by email and may register for the meeting.

PLEASE NOTE: Guests who have not been approved for an invitation may not register for the meeting.
